ORDER MY PET REMEMBRANCE GLASS



TO COMPLETE ORDER FOLLOW THESE STEPS:

1.Fill out the order form and write down your order number on the front of the envelope you send and on any materials submitted in the envelope. (a printed copy of your completed order form should accompany your materials if possible)
2.Place a teaspoon of ash (per piece ordered) in a small ziplock bag.  Please write your name, the name of your loved one and your order number on the bag. *Optional: We welcome you including a short note or photo about your loved one for the artist. 
3.Place the contents in an envelope and mail to:
